I certify that:
- I will comply with all ANSTO regulations.
- I have informed ANSTO of all relevant medical conditions, including pregnancy, pacemakers and serious allergies.
- I am insured against the risk of illness, surgical expenses and industrial accidents.
- The samples to be used and the experimental conditions are as quoted in the proposal.
- When publishing my results I will give proper credit to ANSTO staff and proper reference to the ANSTO facilities.
- I will provide the Bragg Institute with copies of publications resulting from experiments performed at ANSTO.
