What if there was a fire on the truck, or a terrorist launched a missile attack while in transit? 

If such an event occurred, the levels of radiation dispersed would not pose a significant health risk to the general community.  Any health risk from such an incident would be due to non-radiological reasons such as in any fire or explosion.
